Am I a Candidate for LASIK?

Not all patients are suitable candidates for LASIK. The procedure is performed on the cornea, which is very delicate. If your cornea is too steep, flat, thin, or irregularly shaped, it might adversely affect the results of the procedure, ruling you out as a good LASIK candidate.

You must meet the following criteria to be considered a good candidate for LASIK:

Over the age of 18
Must require a prescription that fits in the range established by FDA guidelines
Must be willing to accept all potential risks, complications, and side effects
Vision must be stable for at least one year prior to surgery
Cornea and retina must be healthy
Cornea must be thick enough to undergo laser reshaping
Eye pressure should be normal
Must not have any other major eye conditions
Should not be pregnant or nursing
There should be no evidence of collagen vascular or autoimmune disease
There should be no recent history of herpes in the eye
Must have realistic expectations for the outcome of the procedure
Contact lens use must be discontinued for a minimum of three days before your initial evaluation

The Benefits of PRK

If you are not an ideal candidate for LASIK, then PRK may be the right procedure to correct your refractive errors. If your corneas are too thin, steep, flat, or irregularly shaped, LASIK may not be a viable option for you. However, you may be a better candidate for a surface ablation technique such as PRK which removes less corneal tissue than LASIK.

One of the primary benefits of PRK is that there is no need to create a corneal flap. Therefore, all of the risks and complications associated with flap creation are not present in PRK. Other benefits include:

Allegretto Wave

The Allegretto Wave laser employs advanced technology called PerfectPulse Technology to provide you with greater accuracy in your LASIK procedure. The laser is calibrated to the unique specifications of your eye contour, resulting in a greater degree of control as the laser reshapes your cornea.

How Does IntraLase Differ from LASIK?

Traditional LASIK procedures use a microkeratome blade to cut the flap in your cornea. The creation of the corneal flap is an integral part of your LASIK procedure, and a more precise flap will greatly improve the results. A microkeratome blade introduces human error to the creation of the corneal flap, which may lead to an increased risk of higher order aberrations such as halos, glare, and starbursts.

The IntraLase AMO laser creates the corneal flap using a femtosecond laser. It is a completely bladeless procedure, resulting in a more precise corneal flap, improved safety, and a greater likelihood of achieving 20/20 vision.

IntraLase technology is a minimally invasive technique that utilizes a computer software program to guide the laser during the flap creation. This software provides for greater accuracy in flap creation and a smoother corneal surface.

The use of metal blades may result in uneven flap edges and abnormal corneal surfaces which might contribute to additional vision problems. They have also been known to create an improperly formed “buttonhole” flap which may lead to scarring that interferes with your vision. Flaps created with the IntraLase laser are much less likely to result in these complications.

Kelly Laser Center offers Crystalens intraocular lenses (IOL) as a treatment for patients suffering from cataracts. Crystalens represents the latest in IOL technology and provides an effective method to correct vision problems caused by cataracts.

Intacs vs. Laser Vision Correction

Intacs corneal implants and laser vision correction are very different methods to correct refractive errors in your vision. While they primarily address different eye conditions, they both are effective options to treat nearsightedness and astigmatism.

Intacs are small curved pieces of plastic that get inserted into your cornea to flatten its curvature. Eyes suffering from myopia have a steeply curved cornea that bends light too sharply. Intacs can correct this condition by flattening the curve of your cornea so that light can focus clearly on the retina.

Intacs can also be an effective method of treating astigmatism, which causes blurry vision at all distances due to an oval-shaped cornea. Intacs can be placed in your cornea in a way that returns your cornea to a more spherical shape. However, Intacs are primarily used to treat Keratoconus, a condition marked by a gradual bulging of the cornea.

Keratoconus

Keratoconus is a condition affecting your cornea. Due to fluid exerting pressure on the wall of your eyeball, your cornea bulges forward in the center, becoming thinner and changing from a round shape to a cone-like shape. Keratoconus leads to the following vision issues:

Distorted vision
Blurred vision
Seeing multiple images
Increased sensitivity to light

This is a progressive condition. In other words, it gets worse over time, resulting in increasing levels of nearsightedness and astigmatism.

There are a variety of treatments which can help correct vision issues caused by Keratoconus. New York ophthalmologist Dr. James Kelly will recommend the appropriate treatment based on the severity of your condition:

Contact lenses - These may be able to correct the condition at first, but as your corneal bulging gets more severe, you may need to explore alternative treatments. Rigid gas-permeable (RGP) contacts may help smooth the shape of your cornea, but they are often uncomfortable. You may be able to “piggyback” two pairs of contact lenses – one soft and one RGP - with the soft lenses touching your eyes to increase comfort.
Scleral lenses - These contact lenses are larger in diameter, and as a result, the edges rest outside the cornea on the sclera (white portion of your eye). This reduces corneal pressure.
Intacs corneal implants - These are curved pieces of plastic which get inserted beneath the surface of your cornea. Intacs corneal implants push against the periphery of your cornea in order to flatten the curvature. Intacs are an excellent treatment option once your corneal curvature is too severe to be treated by contact lenses.
Surgery - Once your condition worsens to the point where Intacs are no longer effective, Dr. Kelly can perform a corneal transplant. He can either replace the entire cornea or do a partial-thickness replacement. The surgery should improve your vision, but you may still need corrective lenses for optimum vision.

Intacs Corneal Implants

Intacs corneal implants are thin plastic crescents inserted into the sides of your cornea to correct irregularly curved corneas. They have been approved by the FDA to treat mild cases of myopia as well as Keratoconus, a progressive eye condition which is caused by bulging of your cornea in front of the iris. Over time, this condition gradually worsens, resulting in a steeper curvature at the center of your cornea.

Intacs are constructed of a biocompatible material which rarely interacts poorly with your body. As your Keratoconus gets more severe, you may need to switch out your Intacs with a different size to provide the proper treatment.

The Intacs procedure can be completed in approximately 15 minutes. New York ophthalmologist Dr. James Kelly will administer anesthetic eye drops and a mild sedative before performing the procedure. He will then place your Intacs outside of your visual field so that you cannot see them. The device will be inserted between the sub-layers of your cornea’s stroma (middle layer). Dr. Kelly will choose the appropriate thickness of Intacs based on the visual distortion you experience.

You should notice an improvement in your vision within a day or two. It is important to get proper rest after the procedure to facilitate the healing process. Depending on the severity of your Keratoconus, you may still need corrective lenses after your procedure.

Intacs corneal implants are a highly effective treatment for Keratoconus, and approximately 96% of patients can see well at all distances following the procedure.

Reduce the Chance of Halos or Glare with Allegretto Wave

Some people experience night vision changes after LASIK. They see halos around lights and notice glare more than they did before LASIK. While these side effects occur only in an small number of the hundreds of thousands who receive LASIK every year, they may be more likely if their eye surgery was performed with an older generation laser. The Allegretto Wave excimer laser, approved by the FDA, has reduced this small chance even further.

Allegretto Wave uses PerfectPulse Technology, which means each pulse of the laser is calibrated for your own unique eye. A high level of control means that even though the Allegretto Wave laser beam pulses 400 times per second, there are checkpoints in place to watch the tracking. Even if your eye moves away from the beam, it will stop and wait for your eye to return to position before continuing. Shaping the cornea is done much more precisely than with older lasers.

To add to this, Allegretto Wave uses wavefront-guided technology. Before your LASIK surgery, your eye is mapped down to a microscopic level. This allows the laser to know where it needs to go. When it knows where to go, your corneal sculpting will be much smoother, resulting in clearer vision.
